Job Title: Nurse Case ManagerJob Description We are seeking a dedicated Nurse Case Manager to join our team within a surgical setting in the Orthopedics department. This role is perfect for nurses looking to step back from heavy clinical roles and focus on care coordination and team leadership. Responsibilities + Coordinate care for a select caseload of patients from triage through care coordination during management and safe transfer to home. + Proactively communicate with payors to ensure approval of treatment plans and required resources. + Maintain knowledge of and respond to clinical system and fiscal data related to specific patient populations. + Collaborate with the multi-disciplinary team, patients, and patient families to ensure effective and efficient transition planning. + Coordinate transition to post-acute settings including nursing facilities, acute inpatient rehab, and local shelters. + Manage patients based on departmental procedures while maintaining clinical safety and quality. + Enter and maintain patient information in all departmental systems as per operational guidelines. Essential Skills + Active RN License + 2 years RN experience + BLS Certification + Current in-patient Case Management experience + Experience with post-acute facility referral platforms such as NaviHealth or AllScripts Additional Skills & Qualifications + Experience with EPIC + Case Management Certification + Proficiency with EMR and nursing documentation software + Strong leadership and organizational skills + Tech-savvy with excellent medical knowledge Work Environment This position involves working within a surgical setting in the Orthopedics department. The role consists of five 8-hour shifts from 8:30 am to 5 pm, with no weekends, holidays, or on-call duties. The nurse case manager will primarily work behind a computer, managing the nursing team and ensuring streamlined and efficient patient care. This position offers a more comfortable role within the hospital, with opportunities for leadership career growth. Pay and Benefits The pay range for this position is $35.00 - $45.00/hr.
