We are seeking a Licensed Practical Nurse (LPN) Case Manager  to join our team in our Bethlehem, PA location.

 

The LPN Case Manager will be  responsible for:

coordinating and implementing services for consumers as identified in the service plan.  coordinating the overall medical care for consumers, following through with Doctor’s medication orders. provide education for better understanding of medical conditions, advocacy during medical appointments.  providing supportive counseling, training, and case management services utilizing a flexible schedule on-call responsibilities.

 

Earn $32.44 / hour with the selection of our Enhanced Pay option.
