Nurse (Clinical Accountability & Metrics) - Discharge Planner

This position is located at Kayenta Service Unit (KSU), Inpatient Care Branch, Kayenta, AZ. The primary responsibility of this position is to provide case management and discharge planning services. Incumbent works under the Inpatient Supervisory Nurse. Responsibilities This position is the Nurse (Clinical Accountability & Metrics), Discharge Planner position. Discharge planning is a multidisciplinary approach to ensure the continuity of care for patients. It begins at admission, continues through hospitalization and discharge, and on through post hospitalization care as needed. Responsible for coordinating the available resources by identifying, mobilizing, and utilizing those resources to meet the patient's needs. Makes daily rounds on all units to meet newly admitted patients. Coordinates and conducts frequent discharge-planning meetings on the inpatient unit. Functions as a liaison between patients in the Kayenta Service Unit and/or other health care facilities. Manages all post hospital care and follow-up through coordination of patient's discharge planning with multi-disciplinary teams of physicians, nursing personnel, social workers, physical therapists, etc. Using approved KSU formats, documents discharge planning meeting, activities, plans, policies, and procedures. Maintains a system of tracking referred patients for admission in the other health care facilities and performs chart reviews to detailed timeliness of request services to facilitate the return of patients, or other health care facilities or discharge to home. Participates in case conferences between various disciplines, families and agencies involved in the patient care plans. Works closely with the Utilization Review Coordinator and Contract Health staff for necessary follow-up on claims and identifying long-erm disability patients. Collects relevant data for program evaluation including service utilization, identified problems and status of resolutions, patient satisfaction, etc. Participates in hospital Contract Health review with Peer Review Organization (PRO) when contract renewal comes up or as addendum is necessary. Serves as the team leader of the interdisciplinary healthcare team promoting the coordination, collaboration, and continuity of care from preadmission to post-discharge from a variety of Health care providers. Functions as a relief Tour Coordinator and oversees employees in all clinical area. Functions as Patient Advocate to ensure good Guest Relations throughout the facility. Conducts exit interviews with patients and distributes questionnaires to assess our guest's perception of the quality and appropriateness of the care they received in our facilities. Qualifications To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. BASIC REQUIREMENT(S): Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, The District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. GS-11: In addition to the above requirements, applicants must meet one of the following requirements: Successful completion of a PHD or equivalent doctoral degree from a professional nursing educational program or related medical science field; OR, At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing. GS:11 specialized experience include: Case Management and Discharge Planning functions, coordination, and implementation of a multidisciplinary process to plan for the continuing care needs of a patient following discharge from the hospital; collaborates with nursing, medical staff, social services, physical therapy, mental health, public health nursing, contract health service, patient education and others to provide a comprehensive discharge program for patients; develops and implements comprehensive plans for safe discharge of patients from any alternate health care facility; responsibilities related to clinical case management activities necessary to purchase patient care in the community; experience as a specialty care management manager to ensure the continuity of care for patients through all forms of care including but not limited to the Emergency Department, Patient care Units, and outside facilities.
