Position Summary:
We have an exciting opportunity to join our team as a Nurse Coordinator at the Pearl Barlow Center for Memory Evaluation & Treatment. 

This patient-focused position is responsible for high-quality communication and care coordination for patients of the NYU Langone Pearl Barlow Center for Memory Evaluation or Treatment. The Primary focus will be supporting clinical communication and coordination between patients and clinical teams This position will work closely with the practice physicians and APPs This includes but is not limited to patient screening and clinical intake following up on testing coordination and results appointment scheduling replying to patient advice requests and questions and ensuring patients are receiving timely and clear responses throughout their care episodes The position collaborates with internal and external providers as well as other NYULMC staff and partners to help achieve high patient satisfaction for patients/families.

Job Responsibilities:

Partners with practice clinicians and staff to ensure high-quality patient communication and consistent patient satisfaction.Contacts patients who are interested in infusions and discuss the medication administration protocols, risks, and benefits.Conducts chart reviews for patients who would like to pursue Leqembi infusion.Keeps track of the infusion timeline for patients who are currently receiving Leqembi Infusion.Assists physicians with consenting appointments: answering any questions regarding logistics/protocols.Acts as a professional role model to all levels of staff by considering the needs and behaviors of specific patients in a culturally competent manner and incorporating expertise, critical thinking, and related experience in the care of patients and families.Provides for the educational needs of self, patients, and administrative staff. Participates in the development and upgrading of patient/family education materials and programs as relevant to expertise and practice.Participates in quality improvement programs and activities that promote patient, family/caregiver, physician, and staff satisfaction.Acquires and maintains current clinical knowledge and competence to fulfill responsibilities.Interacts with clinical and administrative positions in a variety of roles (e.g., practice staff, physicians, clinicians outside of the practice or NYU health plans).Adheres to department and FGP policies and procedures.Uses and documents in the Epic electronic medical record regarding communication and care coordination activities. Organizes and presents pertinent both normal and abnormal data in a manner meaningful to the physician for medical diagnosis and treatment.Participates in the coordination of practice activities to provide patient care in a healing, humane, and caring environment.Other duties as assigned.

Minimum Qualifications:
To qualify you must have a Licensure as a Professional Registered Nurse in NYS with a current registration BSN or graduate of an accredited RN program with a bachelor's degree in a related healthcare field. Relevant clinical experience in ambulatory care managed care or acute medical surgical experience in the care of the population targeted for care coordination. Neurosurgical or surgical practice experience preferred. Membership in pertinent professional organizations is preferred. Fluent and strong written and spoken English language skills necessary. Possesses demonstrated strengths in interpersonal communication customer service negotiation conflict management creative problem solving clinical leadership change management work process organization and time management. Able to apply critical thinking and clinical expertise toward the achievement of specific outcomes. Previously established ability to foster strong collaboration with co-workers peers physicians nursing and ancillary departmental support staff. Working knowledge of Microsoft Office Outlook Word Excel and PowerPoint and demonstrated proficiency in managing an electronic medical record such as Epic or a care/case management system. 

Required Licenses: Registered Nurse License-NYS

Preferred Qualifications:
Two or more years in an ambulatory health plan or home health setting or ambulatory setting care/case management experience. Membership in care/case management and/or related professional organization Certification in Case Management. American Board of Quality Assurance and Utilization Review Physicians Inc ABQAURP or Certified Professional Healthcare Quality CPHQ Epic proficiency Masters of Science in Nursing or relevant health-related field

NYU Langone Health provides a salary range to comply with the New York State Law on Salary Transparency in Job Advertisements. The salary range for the role is $94,323.65 - $138,005.27 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital needs. The salary range or contractual rate listed does not include bonuses/incentives, differential pay, or other forms of compensation or benefits.
