Pay Rate (Annualized): $70,015.40
The Nurse Evaluator is a salaried position (minimum of 35 hours a week) which works 50% on both the Nursing Home Transition Waiver and the Traumatic Brain Injury Waiver Programs. There are ten regional centers across NY State that administrates these programs in department of health designated regions. . These programs are the responsibility of New York State’s Department of Health. Headway of WNY is the contract holder for the Buffalo Region which included the counties of Erie, Niagara, Chautauqua, Cattaraugus, Wyoming and Orleans.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
The Nurse Evaluator will be a key member of Headway’s Senior Leadership and provide expertise in Headway’s services and growth, providing valuable information from a medical perspective. As such, it is expected that the Nurse Evaluator be current in medical information relevant to the mission of Headway of WNY. The Nurse Evaluator will be fully trained as a UAS assessor level 45 as well as manager level 50 which will assist the director of Headway in reviewing assessments, workflow and administrative reports for Headway of WNY to the Health Commerce System in New York. The Nurse Evaluator will help ensure the standards and usage of the Health Commerce System by Headway and its trained employees meets the standards of the NY State Department of Health. In addition, the Nurse Evaluator must also maintain certification in performing the Patient Review Instrument and its accompanying Screen. The Nurse Evaluator is available to consult/assess/review and report to the Regional Resource Development Center at Headway of WNY (RRDC) and the New York State Department of Health (DOH), on most cases assessed by the RRDC for both the NHTD and TBI waiver programs, and perform assessments of possible waiver participants for both programs in person or by telephonic or telehealth linkage and in a variety of locations including but not limited to the potential waiver participant’s home, nursing home, hospital or rehab center. Complete assessments of individuals referred to the Waiver program(s) and makes recommendations to the RRDC based on the requirements of contracts (regulations) with the NYS Department of Health, that take into account the needs of the participants as per consult with the Director and Lead RRDS’s. Review and comment on acquired medical information for referrals, applicants and participants in both waiver programs Respond to calls from individuals with disabilities and seniors, family members, advocates, professionals and others requesting information regarding waiver services. Completes very detailed reports and maintains clear communication with formal supports, potential participants and informal supports in institutions and the community. Work with waiver team to tracks referrals and intakes and helps maintain an informational database vital to the analysis of the effectiveness of the waiver programs. If requested attend meetings in Albany and the region. Follow up with referrals by phone, fax or electronic communication. The Nurse Evaluator must be able to present on nursing, community long term care issues and waiver issues. Nurse Evaluator will be involved in the marketing of the program. If requested present on behalf of the RRDC at fair hearings. If requested train staff and providers on relevant waiver and medical issues Be able to drive and maintain a means of transportation. Have basic knowledge of e-mail, internet and Microsoft Office. Complies with all agency policies and procedures. Other duties as assigned.
MINIMUM QUALIFICATIONS:
Registered Nurse with valid current RN license in New York State. PRI/Screen and UAS certified or able to qualify and complete needed training and certification within reasonable time of hire. Have a minimum of three years RN experience with at least two of those years working as a community nursing with experience working with individuals with disabilities, seniors and TBI. Valid driver license Able to drive and travel regularly in the six county region of the Buffalo RRDC Be available to attend meetings if requested in New York State Be a team player who works well with other staff Able to use Microsoft Office Word, and type reports Possess excellent communication skills Be able to present to groups
