Nurse-Family Partnership (NFP) is a national evidence-based nurse home visitation model that pairs nurses with low-income first-time mothers, beginning in pregnancy and continuing until the child’s 2nd birthday. The NFP nurse home visitor position offers a highly rewarding experience for nursing professionals interested in improving the health and social outcomes of first-time mothers/babies in Central Brooklyn. Responsibilities include:
Attend required in-person training in Denver, Colorado within 4-6 weeks of hire.
Develop and maintain a caseload of 25 families over the first year in the position.
Deliver culturally sensitive services to a diverse population.
Maintain fidelity to the NFP model re:
Frequency, content, and documentation of services.
Adherence to program and agency policies and procedures.
Qualifications
BA in Nursing required.
Current RN license (in good standing) required.
Minimum of two years recent experience in maternal/child health, pediatrics, and/or community health required.
Excellent written and verbal communication skills
Basic computer skills
Strong organizational/time management skills
Spanish, French and Creole language skills a plus.
