Plans, supervises, evaluates, and coordinates the delivery of home health services to participants in collaboration with the Director and in accordance with the policies and procedures of Rochester Regional Health and ElderONE. Develops nursing policies and standards for the delivery of care. Focuses on customer service as it relates to patients, visitors, family members, staff and physicians including working collaboratively with other disciplines to ensure an efficient, team approach to the delivery of patient care. Assures compliance with NYSDOH, LHCSA and PACE regulatory requirements. Responsible for the direct supervision of RNs, LPNs and Home Health Aides in accordance with Home Care standards. Provides guidance for staff members in developing appropriate expectations and outcomes.

ATTRIBUTES
Current New York State Registered Nurse License.ASN required Bachelors in Nursing preferred.Homecare experience preferred, but not requiredMinimum 2 years' Nursing Experience preferredMinimum 1 year of Supervisory experience preferredWorking knowledge of NYSDOH LHCSA or CHHA regulations and Federal Conditions of Participation preferred.1 year of experience working with a frail or elderly population preferred.
RESPONSIBILITIES
Leadership & Management. Assume responsibility for overseeing nursing and non-nursing personnel on a unit; function as a unit leader with the ability to act as a resource (both clinically and non-clinically) for nursing personnel, physicians and providers; delegate appropriate responsibilities to assigned nurses and ancillary personnel; assess clinical expertise of nursing personnel, monitor assignments and provide feedback and documentationGeneral Management. Perform daily leader rounding and assure team members are regularly rounding on all patients; develop programs and strategies to monitor the quality of patient care and patient outcomes; assist Nurse Manager in developing and implementing long-term staffing pattern, personnel needs, budget negotiation and recruitment planEDUCATION:

BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.

14617The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
