Position Summary

The Nurse Manager shall be responsible for the direction, provision, and quality of nursing and patient care services provided to hospice patients and shall work collaboratively with the Director of Hospice to ensure collaboration and coordination of care among patient care disciplines.

Primary Position Responsibilities

1. Overall planning, supervision, and administration of nursing services;
2. Coordination and integration of nursing services with other hospice services to provide a continuum of care for the patient;
3. Development of protocols for regular verbal communication, including case conferencing, between the nursing services and other disciplines based on the needs of each patient;
4. Development of written job descriptions and performance criteria for nursing personnel, and assigning duties based upon education, training, competencies, and job descriptions;
5. Ensuring that nursing services are provided to the patient as specified in the nursing plan of care.
6. Responsible for clinical and documentation compliance in conformity with federal, state, and Hunterdon Hospice/Health policies governing hospice services. Actively participates in the agency’s QAPI program.
7. Other duties as assigned.

Qualifications

Minimum Education: Required: A Master’s degree in nursing or a health related field and two years combined public/community health nursing and progressive management experience in public health; or a bachelor of science degree in nursing or a health related field and three years combined public/community health nursing and progressive management experience in public health nursing. Minimum Years of Experience (Amount, Type and Variation): Required: Two years Home Care, Hospice or other public/community health nursing experience Preferred: Hospice experience
Management experience
License, Registry or Certification: Required: New Jersey License as Professional Registered Nurse Preferred: Certified Hospice and Palliative Care Nursing (CHPN)
Knowledge, Skills and/or Abilities: Required: Knowledge of Hospice philosophy, symptom management and regulations; good interpersonal skills; strong organizational skills and clinical management skills Preferred: None
