Job Title: Nurse Navigator Job Description As a Nurse Navigator, you will be the primary point of contact for patients, guiding them from their first day of treatment. Responsibilities include reviewing treatment processes, managing side effects, providing post-treatment follow-ups, and offering triage and symptom management. You will also facilitate patients' transition from hospital to home, maintain treatment areas, monitor inventory, and coordinate care plans through phone communications. Work Site This is a fully on-site position in Schaumburg, Illinois. Work Environment PT ROLE - 3x/week on-site (first-shift hours 8am-430pm) Join a bustling treatment center that serves 120 patients daily. You will work closely with Infusion RNs, other healthcare providers, and CMAs, ensuring comprehensive care delivery. Work hours are three days a week from either 8 AM to 4:30 PM or 8:30 AM to 5 PM with no weekends or holidays.
