Summary:
The Nurse Navigator demonstrates critical thinking and uses the nursing process to assess and meet the needs of cancer patients by providing care coordination through the cancer continuum. This position acts as a liaison between patients, families and caregivers and the healthcare delivery system to optimize patient outcomes while reducing barriers to care.
Responsibilities:
1. Coordinates designated administrative operations for the section, under the general direction of the Director. Supports and upholds departmental policies and standards regarding patient flow and operations by working with departmental staff to maintain throughput processes.
2. Coordinates Multidisciplinary Program.
3. Coordination of Care.
4. Communication: Provides psychosocial support to and facilitates appropriate referrals for patients, families and caregivers, especially during periods of high emotional stress and anxiety. Builds therapeutic and trusting relationships with patients, families and caregivers through effective communication and listening skills. Facilitates communication among members of the multidisciplinary cancer care team to prevent fragmented or delayed care that could adversely affect patient outcomes.
5. Participates in Development of Standards. 6. Participates in Community Outreach Activities. 7. Displays Leadership. 8. Performs Miscellaneous Duties.

**Education Requirements:**
• Bachelor’s Degree in Nursing.
• Master’s Degree in Nursing is preferred.
**Licensure/Certification Requirements:**
• Current license to practice as a Registered Nurse in North Carolina.
• BLS Certification.
**Professional Experience Requirements:**
• 3 years’ experience Healthcare/Medical - Nurse: Registered Nurse In an acute care setting Supervisory/management development course work, training, or experience.
• Prior experience in working directly with oncology patients is preferred.
