Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses. McLeod Health is also composed of approximately 15,000 team members and more than 90 physician practices throughout its 18-county service area. With seven hospitals, McLeod Health operates three Health and Fitness Centers, a Sports Medicine and Outpatient Rehabilitation Center, Hospice and Home Health Services. The system currently has 988 licensed beds, including Hospice and Behavioral Health. Essential Functions
Provides education to patients and family members about specific disease processes. Serves as liaison between patients and clinical care providers, facilitating communication and coordination of care as needed. Participates in care conferences. Develops and provides patients with comprehensive treatment summaries/survivorship plans of care upon completion of treatment. Promotes the profession of nurse navigation through patient quality improvement initiatives. Maintains working knowledge of evidence-based practice regarding screenings, surveillance, therapies, and treatments for the specific disease for the population served. Develops and maintains professional relationships with physicians/care providers and other professional colleagues. Under the direction of supervisor, assist with reporting quality metrics for specific population served. Participates in community events, such as community health fairs, as requested.
Required Education/Experience
Graduate from an accredited registered nursing program Must possess a valid South Carolina or compact (multi-state) RN license. This position requires an active Basic Life Support certification, as well as additional certifications per unit requirements. Minimum of 3 years of nursing experience.
