Epilepsy Center in the Ayer Neuroscience Institute is highly dedicated to improving the lives of our patients We believe good communication and mutual respect is at the heart of any good team and is essential to providing the best care Our level IV epilepsy center has all the diagnostic and treatment modalities available in order to provide cutting edge care for people with seizures We look forward to welcoming a nurse navigator to assist us in coordinating the care and improving the efficiency with which the people of Connecticut can access these outstanding tools We envision our nurse navigator as a central aspect of our program interacting with patients clinicians and staff across the continuum of care starting with the first clinic visit and leading in some cases to brain surgery to eliminate seizuresJob ResponsibilitiesServes as a member of the multidisciplinary care team functioning as a patient advocate educator key point of contact and liaison between the patient and all other appropriate services Navigates patients through the continuum of care working with our social worker faculty and staff to eliminate barriers and enhance patient and colleague experienceUtilize electronic medical record reports and data management skills to coordinate patient flow through the epilepsy surgery programIdentify patients with seizures within Hartford HealthCare who would benefit from referral into epilepsy clinicsParticipate as need in community and colleague education initiativesAssist in tracking and reporting of program performance measures quality indicators and responsibilitiesKnowledgeable regarding data interpretation and the tracking and evaluation of quality indicatorsDemonstrates use of clinical skills and expertise in the area of diagnosis treatment and psychosocial impact of diseaseDemonstrates excellent customer service skills including superior telephonic communications skills with the ability to comprehensively assess patientrsquos physical and psychosocial status as well as conduct complex medical history discussions over the telephoneAbility to effectively and efficiently coordinate implement and monitor the patientrsquos process of care to communicate openly and empathetically with patients and their family membersAbility to work independently as well as collaboratively with navigation team and multidisciplinary care team in all aspects of defining and implementing navigation program Ability to communicate effectively with diverse audiences including professional staff patients community media and other service agenciesKnowledge of adult learning competencies and patient teaching skillsDemonstrated management skills reflecting self motivation initiative and fiscal responsibility Tolerance of ambiguity and
