essential to providing the best care Our level IV epilepsy center has all the diagnostic and treatment modalities available in order to provide cutting edge care for people with seizures We look forward to welcoming a nurse navigator to assist us in coordinating the care and improving the efficiency with which the people of Connecticut can access these outstanding tools We envision our nurse navigator as a central aspect of our program interacting with patients clinicians and staff across the continuum of care starting with the first clinic visit and leading in some cases to brain surgery to eliminate seizuresJob ResponsibilitiesServes as a member of the multidisciplinary care team functioning as a patient advocate educator key point of contact and liaison between the patient and all other appropriate services Navigates patients through the continuum of care working with our social worker faculty and staff to eliminate barriers and enhance patient and colleague experienceUtilize electronic medical record reports and data management skills to coordinate patient flow through the epilepsy surgery programIdentify patients with seizures within Hartford HealthCare who would benefit from referral into epilepsy clinicsParticipate as need in community and colleague education initiativesAssist in tracking and reporting of program performance measures quality indicators and
