Nurse Navigator - Neuroscience

Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses. McLeod Health is also composed of approximately 15,000 team members and more than 90 physician practices throughout its 18-county service area. With seven hospitals, McLeod Health operates three Health and Fitness Centers, a Sports Medicine and Outpatient Rehabilitation Center, Hospice and Home Health Services. The system currently has 988 licensed beds, including Hospice and Behavioral Health. The hospitals within McLeod Health include: McLeod Regional Medical Center, McLeod Health Dillon, McLeod Health Loris, McLeod Health Seacoast, McLeod Health Cheraw, McLeod Health Clarendon and McLeod Behavioral Health.  

1.    Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.
2.    Functions as a part of the multidisciplinary Neuroservices team for provision of care to the Neurology, Stroke, Neurosurgical and Thrombectomy patients.
3.    As needed, navigates Neuroservices patients through the trajectory of care by assisting with facilitating patient appointments to care providers.
4.    As needed, provides education to patient and family members about specific disease processes.
5.    Develops and provides patients with comprehensive treatment summaries/survivorship plans of care upon completion of treatment.
6.    Serves as liaison between patients and clinical care providers, facilitating communication and coordination of care as needed.
7.    Participates in site specific Neuroservices conferences.
8.    Develops and provides educational needs to physician and staff for Neuroservices.
9.    Promotes the profession of  Neuroservices navigation through patient in quality improvement initiatives within the Neuroservices program.
10.    Maintains working knowledge of current therapies and treatment details for the specific disease for the  Neuroservices population served. Demonstrates competency in, but not limited to the following (refer to McLeod Health Competency Checklist):

•    Patient Education   
11.    Develops and maintains professional relationships with physicians/care providers and other professional colleagues within the Neuroservices division.
12.    Is knowledgeable of and adheres to DNV Stroke accreditation standards.
13.    Participates in Neuroservices Committees as requested.
14.    Participates in community events, such as community health fairs, as requested.
15.    Represents the organization in a positive and professional manner at all community events. 
16.    Completes annual training and annual educational requirements timely.
17.    Performs other tasks, duties or responsibilities as assigned or need for the effective, safe and efficient functioning operation of the hospital.

Qualifications/Training:
•    Excellent communication skills – verbal and written
•    Ability to work independently

Licenses/Certifications/Registrations/Education:
•    Must possess a valid SC license – RN, BSN
•    Advanced Certification as approved by AVP or VP within 1 year of hire
