patients receiving care within the defined Service Line This includes working collaboratively with other health providers to assure coordinated care throughout the service linersquos continuum of care The nurse navigator will assist with the coordination of services and will provide support and education throughout diagnosis treatment and into survivorship for patients Will serve as a professional resource in the areas of the Neurosurgery Service LineAct as a liaison between healthcare providers payors and the patient familyAssist patients with the scheduling of appointments for follow up visits with other healthcare providersContact patients instructing them of additional imaging procedures that must be performed to clarify interpretationServe as a consistent contact for patient family throughout the process of diagnosis and treatment providing pre procedure teaching and post procedure follow up callsWork with providers on treatment plans follow up care and surgical optimizationDevelop coordinate and continually improve nursing procedures and processes pertaining to the Neurosurgery Service Line coordination of careParticipate in outcomes research and tracking
