Nurse Navigator - Radiology - The Breast Center
**Hours of Work :** varies **Days Of Week :** Monday - Friday **Work Shift :** **Job Description :** Your Job: The primary purpose of the Nurse Navigator position is to provide continuity and coordination of patient care; conducts patient and professional education initiatives; holds a collaborative relationship with multidisciplinary teams, community resources and agencies. Your Job Requirements: • Graduate from an accredited school of nursing • Licensed in Texas as a Registered Nurse. • Current CPR certification • Specialty Oncology certification preferred Your Job Responsibilities: • Communicate clearly and openly • Build relationships to promote a collaborative environment • Be accountable for your performance • Always look for ways to improve the patient experience • Take initiative for your professional growth • Be engaged and eager to build a winning team • Assesses plans, develops, implements and evaluates formal and informal educational opportunities and materials for patients, families and the community regarding breast cancer and breast cancer awareness. Educates breast cancer patients and families about their individual breast cancer diagnosis. Navigates each patient and their family through the treatment continuum by communication and coordinating with referring physician, surgeons, oncologist and multidisciplinary team. • Collaborates and communicates with diverse medical staff, physician office practices, cancer data management and ancillary departments of the Methodist Health System to evaluate and improve patient outcomes and to collect data for performance improvement initiatives. • Collaborates with Public Relations to educate referring physicians, surgeons, oncologist and other facilities on our available services. • Facilitates support programs/focus groups, community resources, grants and/or refers patients and families to the appropriate cancer support programs and resources that are available in their community. • Serves as a clinical resource/consultant to provide up to date information and answers to questions pertinent to breast cancer. Determines appropriate resources for breast care center, ancillary departments, physician’s practices, nursing units and the community/public. • Other duties as assigned.
