Responsibilities
The Nurse Practitioner has advanced certification in hospice and palliative care to act as a resource to nurses and medical providers in providing expert hospice and palliative care to patients of all ages.
Essential Key Job ResponsibilitiesAssists in various strategies and acts as a resource to the MBO in growing the home health, hospice, and palliative care servicesProvides Face to Face visits for hospice recertification for specific agencies as assignedProvides education to staff including education related to hospice and palliative care as deemed necessary to aid in the development of agency staff Provides recommendations and guidance for management of complex patient symptom management of hospice and palliative care clientsResource for Goals of Care conversationsProvides chart review and education on concise, complete documentation in the medical recordAttends agency meetings to provide input and education as neededInvolved in local, state and national organizations to improve quality of care for patients with serious illness, including hospice and palliative care clients ( ECHO, CPC+, Great Plains MN/ND)Participates in agency IDG meetings as able or when requestedProvides general community education and education to the medical community about palliative careActs as a resource for clinical coordinators in decision-making for complex patient situationsQualifications
Graduate from an accredited school of nursing followed by graduation from a recognized nurse practitioner program
Current NP license in good standing to practice in the state or multiple states is required.
Certification as Advanced Certified Hospice and Palliative Care Provider (ACHPN) or obtain certification within 2 years of employment.
Must have valid driver’s license, auto liability insurance according to company policy
Minimum of two years experience
