Nurse Practitioner, Saint Joseph PACE
**Employment Type:** Full time **Shift:** Day Shift **Description:** Nurse Practitioner Saint Joseph PACE Position Purpose: + Collaborate with PO physicians to deliver primary medical care, assessments, and care plans for participants. Serve as a vital member of the Interdisciplinary Team (IDT), ensuring effective communication and documentation of participant health. What you will do: + Provide primary medical coverage, assessments, and care planning in conjunction with PO physicians. + Assist in discharge planning and manage acute illness, prescribing medications and therapies as needed. + Conduct nursing home or home visits as necessary. + Collaborate on clinical policies and standards of care, offering guidance and training to staff. + Advise on improving accountability and participant services, serving as a resource during program operations. + Participate in provider on-call rotation. Minimal Qualifications: + Master's degree in Nursing. + 5-7 years of experience, preferably in long-term care. + Current, active license to practice in IN, along with CDS Certification and DEA Registration. + Board Certification by ANCC or AANP, plus certification as a Nurse Practitioner with prescriptive authority. + Current BLS certification and proficiency with Microsoft Office suite. + Valid driver’s license and ability to travel within the region. + Experience working with the elderly population preferred.
