Nurse Practitioner
Overview

Provides advanced nursing clinical care to patients in the Hospice/Palliative Care program consistent with VNSNY philosophy and standards of nursing practice in relation to VNSNY goals and objectives and improves hospice utilization for hospice eligible patients. Works under general supervision.


Responsibilities
Provides advanced nursing care and treatment to hospice patients in both the home and via telephonic visits, which includes assessment, diagnosis, prescribing medication, ordering and interpreting appropriate tests, and ordering treatments and DME. Performs in-person pre-admission F2F visits when necessary, for certification of terminal illness by the Hospice Team Physicians and complete hospice admission to the Hospice Program when deemed hospice eligible. Perform F2F visits for re-certification at specified intervals for continued service in the Hospice Program.Completes certification of death.Provides primary medical and nursing care which includes prescribing medication to select patients, in accordance with collaborative practice agreement and within scope of practice of a nurse practitioner.Documents patient care services in accordance with VNSNY standards. Collaborates with patients, families, primary care physician and other team members to provide assessment and care planning.  Consults with the primary care physicians and specialists in the community, when necessary, in relation to management and disposition plans.Works with multidisciplinary team (e.g., nurses, social workers, physicians, etc.) on a consistent basis in order to provide collaborative care and make recommendations on additional services (home care, community services, etc.) as necessary. Participates in the on-call NP coverage rotation after hours and weekends for in-person pre-admission F2F visits with completion of admission and completes admissions without required pre-admission F2F visits. Travels to multiple regions visiting patients and families.Documents patient care services in accordance with VNSNY standards.  Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided. Provides care in accordance with VNSNY policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, couching, bending and stretching to deliver patient care.Participates in special projects and performs other duties as assigned.
Qualifications

Licensure:  Licensed and currently registered to practice as Nurse Practitioner in New York State required.  DEA license required.  Valid driver's license may be required, as determined by operational/regional needs.

Education:  Master’s Degree in Nursing required.  

Experience:  Minimum of three years clinical nursing experience required.  Experience as a Nurse Practitioner preferred.
