We are a quality-focused and population-health driven, independent office looking to grow with a Primary Care Nurse Practitioner position. We primarily serve patients 18 years and older, with a patient population having a wide range of healthcare needs, including geriatric and chronic conditions. By providing comprehensive, accessible, whole-person primary care, we are guiding patients to a healthier life, while reducing the cost to the healthcare system. The NP will provide preventive and sick care in an office setting, maintain accurate clinical records in our EHR system, and utilize Accountable Care Organization (ACO) tools to assist patients in accessing high value healthcare.Our Expectations:Provide Direct Patient Care to patients, including plans for comprehensive careDevelop trusting relationships with patientsMonitor the patient’s progress over time with collaboration from the PhysicianClearly communicate care plans to patients and familiesCompletely document progress notes with ICD-10, CPT II, and HCC codingInteract with co-workers in a positive and supportive team-focused mannerWhat We Are Looking For in Our Candidate:An experienced, licensed NP -OR- NP graduate with an active license, or in the process of licenseInherently motivated person willing to work hardOpen communicatorStrong written and interpersonal communication skillsAn interested and active learnerStrong collaboratorTeam oriented and a positive attitudePhilosophically aligned to ACO goals of high quality, high value carePlease note this position is posted on behalf of our partner practices.
