Overview St. Catherine of Siena Nursing and Rehabilitation and St. Catherine of Siena Hospital are proud members of Catholic Health and have served the residents of the Smithtown area for more than 50 years. St. Catherine's is located on the north shore of Long Island in Smithtown. The campus is comprised of a 240-bed nursing and rehabilitation care center; 296-bed not-for-profit community hospital and a 60,000 square foot medical office building. Job Details Certifications: Licensed by the New York State Department of Health Certified by the Medicare and Medicaid Program Operated by a New York State Licensed Nursing Home Administrator DUTIES: Coordinates with the Medical Director, Attending Physicians, Nursing Staff, Rehabilitation Staff and Ancillary Support Staff in providing the direct and collaborative treatment plan for sub-acute, rehabilitation and long-term care residents. In collaboration with the Medical Director and Attending Physicians, performs physical examination, comprehensive patient assessment, medical diagnosis and prescribes appropriate treatment modalities for this resident population. Ensures that all tests, therapies, medications and treatments are initiated, followed and evaluated timely. In collaboration with the Director of Nursing, monitors that the Nursing Home's policies & procedures and nursing standards of care are appropriately followed and maintained. Reviews/audits nursing assessment, planning, intervention, evaluation and medical record documentation for regulatory compliance. Provides resident, family and staff education and participates in Performance Improvement. Demonstrates care that reflects an understanding of individual cultural and religious diversities and needs, and recognition of patient rights. QUALIFICATIONS: Graduate of a Masters in NP program Nurse Practitioner License NP Board Certification BLS and ACLS certification Prior experience in acute, sub-acute, rehabilitation and long term care preferred. Salary Range USD $160,000.00 - USD $170,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
