Position Overview
The Nurse Practitioner (NP) reports to the Medical Director, with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet Care Medical Practice’s (CMP) requirements. They should embody CMP’s core values, including, Trusted, Empathetic, Committed, Humble, Creative and Community-Minded. At CMP, we don’t have patients or customers – we have Family Members.
Key Responsibilities
The NP will have the following responsibilities:
In partnership with the clinical team of CMP, provide medical highest quality medical care for the practice’s patients.In partnership with the Medical Director and Management, assist and support clinical personnel (e.g., clinical nurses, allied health professionals, EMR, telemedicine operations).Provide consultations and perform diagnostic and therapeutic procedures in their clinical discipline within their practice scope, consulting when appropriate the supervising physicians for assistance with both diagnosis and treatment.Works collaboratively with a multi-disciplinary team to create individualized treatment plans for patients; embrace and promote a patient-centric model of care.Commit to the practice of value-based care which emphasizes care in the appropriate setting to improve and lower costs, address and close gaps in care and drive high patient satisfaction.Demonstrate proficiency and support use of the practice management system and electronic medical record (EMR).Perform other job-related duties as assigned.General Duties
The NP will have the following duties:
Leadership: The NP will lead in defining and executing strategies and solutions to create business value in clinical practice, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes.Strategy: The NP will establish the business strategy and roadmap: improve outcomes for CMP’s Family Members; enhance the efficacy of the practice. In establishing the business strategy, the NP will define and innovate sustainable revenue models to drive profitability of the Company.Collaboration: The Nurse Practitioner will ensure that our clinical capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of the practice.Knowledge: The Nurse Practitioner will provide subject matter expertise and clinical solutions, including determining and recommended approaches for highest quality medical care, solution deployment and performance evaluation.Culture: The NP is accountable for creating a productive, collaborative, safe and inclusive work environment for the clinical team and as part of the larger Company.Required Qualifications
The NP should have the following qualifications:
Education: Nursing Degree or Nurse Practitioner Degree from an accredited U.S. school is required. Nurse practitioners must have completed accredited education and training for nursing and nurse practitioners. An active unrestricted medical license in the State of New York is required.Experience: At least 3 years of active practice required. DEA Certification required.Entrepreneurial: Care Medical Practice seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. The Nurse Practitioner will possess the ability to perform certain in-office procedures safely in an outpatient setting.Communication: Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner.Relationships: Ability to build and effectively manage relationships with patients, the community business leaders, and external constituents.Culture: Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Company.Benefits
