Description: Job Summary:
Plans, develops, implements and evaluates activities to optimize outcomes for electrophysiology (EP) patients. Coordinates collection and analysis of data as it relates to electrophysiology patient population. Collaborates with physicians, nurses and support staff to implement quality initiatives to optimize patient outcomes. Manages care for electrophysiology inpatients and outpatients, including assessment, evaluation, education & development of individualized plan of care. Orders and monitors diagnostic tests. Recommends lifestyle modification. With prescriptive authority, may order and adjust medications. Participates with physician in developing strategies for alternative care options.
Essential Responsibilities:
Develops, plans, coordinates, and implements and evaluates regional electrophysiology programs designed to improve care to include primary, secondary and tertiary care services delivered in hospital, out-patient, home and community settings. Monitors and reports status and progress of same to Cardiac Service Line.
Serves as point of contact for Electrophysiology Service Programs. Liaisons between outpatient, inpatient, Emergency Department and community-based programs. Communicates regional patient care goals, procedures, process improvements and program changes.
Coordinates collection of electrophysiology data as it relates to patient population, participates in analysis of internal database elements for quality improvement. Implements focus studies to analyze areas of potential process improvement. Collaborates with physician and nursing partners to implement improvements.
Serves as pacemaker / intracardiac defibrillator (ICD) resource for Hawaii Region. Keeps abreast of technological advances in pacemaker/ICD technology and development of continuing education, and care delivery advances. Functions as liaison between physician, staff, patients and other departments across continuum to promote high quality care and service. Interviews device clinic staff and makes recommendations. Provides input in counseling and evaluating technical skills of device clinic staff. Maintains good communication with neighbor island clinics regarding their pacemaker/ICD patients and device maintenance. Educates neighbor island physicians and staff as needed.
Maintains professional, working relationship with Electrophysiology (EP) physician, pacemaker/ICD vendors, electrocardiograph (EKG) staff, cardiac catheterization laboratory staff, hospitalists, nursing and other patient care staff. Provides feedback to staff, Medical Director, physicians, and other departments regarding electrophysiology issues and concerns. Collaborates with Medical Director to develop operation strategies.
As credentialed and privileged and with physician partner, co-manages care for electrophysiology patients including assessment, evaluation, education & development of individualized plan of care. Reviews and establishes diagnoses. Orders and monitors diagnostic tests, performs procedures. Recommends lifestyle modification. Writes admission and discharge orders. Responds to pages from physicians and staff; Writes admission histories and physicals cosigned by accountable physician. With prescriptive authority, may order and adjust medications.
Demonstrates knowledge of disease pathophysiology and all aspects of nursing and medical management. As appropriate, serves as liaison between patient/family and physician/facility.
Performs pre-procedure patient education. Follows inpatients post-procedure. Monitors process of documentation of device implant information into cardiology data management system. Evaluates wound. Performs initial post-implant device check. Ensures antibiotics and temporary ID cards are received. Performs post-implant patient education for pacemakers and ICD.
Documents all care provided to patients including observations, treatments, medications and instructions according to regional standards. Documents all communications and physician orders. Assures clinical record is accurate, legible and complete.
Serves as consultant for other health care providers regarding patient care and management. Participates in patient care conferences with other health care staff to coordinate, discuss and evaluate services provided to patient.
May perform patient care to the extent necessary to maintain clinical expertise, competency and licensing necessary to fulfill job responsibilities and to direct the provision of care on the unit.
Provides direct patient care on an as needed basis. Provides services that are within scope of license and in compliance with all legal, regulatory, and policy requirements relevant to clinical role performed.
Incorporates the KP Nursing Vision, Model and Values through out their Nursing Practice. Basic Qualifications: Experience
Minimum three (3) years of medical/surgical nursing experience.
Education
Masters degree in nursing with specialty in adult, medical-surgical, acute, critical care, or family nursing. License, Certification, Registration Nurse Practitioner-Family Certification OR Nurse Practitioner-Acute Care Certification OR Nurse Practitioner-Emergency Certification OR Nurse Practitioner-Adult-Gerontology Acute Care Certification OR Nurse Practitioner-Adult-Gerontology Primary Care Certification Advanced Practice Registered Nurse License (Hawaii) Registered Nurse License (Hawaii) required at hire National Provider Identifier required at hire Basic Life Support from American Heart Association Additional Requirements:
