Position Summary:

 

The Nurse Practitioner (NP) is a Registered Nurse Extended Class with graduate education in nursing, possessing advanced clinical and theoretical knowledge/skill within the clinical specialty. The NP is a professional who will function in an expanded role and will demonstrate a high level of autonomy as well as expert skill to formulate clinical decisions, manage acute/chronic illness, and promote wellness.

 

 The Center for Complex Diabetes Care (CCDC) is a newly funded program in Ontario designed to address the needs of complex diabetes patients. The CCDC offers inter-professional collaborative case management based care for people with diabetes in a Mississauga and Oakville locations. The NP will lead case management for the clients of this program.

 

The NP will: conduct comprehensive health assessments; diagnose; provide treatment (including diagnostic, pharmacological and non-pharmacological therapies and counseling); develop, implement and document a plan of care in collaboration with family physicians, endocrinologists, and inter-professional team; provide education; refer to / liaise with other community services; and, provide follow-ups as appropriate.  A strong primary care background coupled with experience in chronic diseases such as; mental health, COPD, cardiovascular and renal disease are necessary to support CCDC clients.  In addition, the NP may be involved in other clinical, research, evaluation, leadership and collaborative activities (e.g. developing new standards of care, policies, etc.).

  

The Principal Responsibilities include:

 

Works collaboratively with nursing colleagues, physicians, allied health team and patient/ family to provide advanced nursing practice through physical and psycho-social assessments and development of outpatient care plans and provide case managementImplements care plan and assists with the evaluation of patient outcomes, and co-ordinates outpatient care plan with primary care and other specialistsCombining the roles that fall within the domains of advanced nursing practice including clinical practice, research, leadership, collaboration and consultation.Developing effective partnerships with physicians and other members of the inter-professional team to provide quality care to patients and their families followed by the CCDC.Applying a comprehensive theoretical knowledge base and approved medical directives and/or within the scope of practice in the extended class, the NP works collaboratively and independently to assess and manage patients’ medical care requirements across the continuum of care, demonstrating exemplary clinical assessment and decision making skills.Providing a key leadership role in evaluating quality of care and patient outcomes within the specialty population, in partnership with the physicians and other members of the inter-professional team.Providing leadership in clinical practice through mentoring and participating in formal and informal staff education.Identifying quality initiatives that will strengthen and further develop nursing professional practice.Evaluating the care provided to the patient including maintaining accurate statistics for reporting.

  

Key Qualifications:

 Registration in the Registered Nurse Extended Class with the College of Nurses with clinical focus in primary careExperience in diabetes management is preferredMembership in the Registered Nurses’ Association of Ontario, requiredDemonstrated excellence in practice in primary care for a minimum of 2 yearsDemonstrated experience with completing comprehensive assessments on complex clientsExperience in evaluating quality of care and patient outcomes within the specialty population, in partnership with the physicians and other members of the inter-professional team.Demonstrated effective leadership skillsStrong interpersonal communication skills and ability to establish positive relationships with all staff, clients, families, and community agenciesCurrent Basic Cardiac Life Support (BCLS) certification
