POSITION PORPOSE

The Nurse Practitioner serves as a leader in an ambulatory health care setting, combining the roles of health care provider, mentor, educator, researcher, and administrator. The NP provides health care services under the direction of a Medical Director.  This role is responsible for coordinating the health care plan for the assigned patient population by collaborating with consulting physicians and health care professionals.  The NP carries out physician orders and maintains the continuum of care through the coordination and integration of all phases of patient care, guided by progress notes, consults, care plans, critical pathways, managed care guidelines, best practice protocols, evidenced based care, and utilization review processes.  In partnership with the supervising Physician, the NP evaluates care based on quality and core measures, access, and cost effectiveness.

This position is a Full-Time position scheduled 40 hours per week.

Trinity Health’s Non-Oncology Infusion Centers in Brighton, MI, is redefining infusion therapy for patients with chronic conditions. Unlike traditional hospital-based infusion bays or oncology-focused centers, our facilities specialize exclusively in non-oncology services, offering a modern, calming environment designed to reduce anxiety and prioritize patient comfort.

WHAT YOU WILL DO

Assesses health status by obtaining a relevant health and medical history, performing a physical examination based on age and history, and identifying health and medical risk factors.

 Performs interventions based upon established priorities which are consistent with the appropriate plan of care, based on scientific principles, theoretical knowledge, and clinical expertise, and consistent with teaching and learning opportunities.

Provides onsite supervision of both clinical and administrative operations, inclusive of policy improvement and operationalization.

Prepares, documents, dispenses, and administers infusion medications and injection therapies in accordance with the order, drug package insert (PI), internal policies, and insurance authorizations.

Meticulously documents findings from patient discussions, visit notes, vital signs, and patient health histories in the electronic health record.

With the assistance of RNs, provides generalized drug education (i.e., side effects as appropriate) to patients and caregivers.

Serves as a reliable, clinical point of contact for referring providers on clinical questions and requests such as: updated order requests, order clarification, patient related questions, and drug related questions.

Maintains accurate, legible, and confidential records.

Provides in-services to staff in response to identified learning needs of staff.

MINIUMUM QUALIFICATIONS

Education:

Minimum:

Active RN and FNP licenses in the state of Michigan; NP License must be Board Certified through ANCC or AANP

Active BLS CPR certification

 Preferred:

Specialty Life Support (i.e., ACLS, PALS, NRP.).

Experience: 

Minimum:

Two years’ experience as a NP or five years practice in Nursing.

Preferred:

Three years’ experience in specialty area with infusion experience.

Two+ years of infusion experience with a familiarity with ports.

Exceptional ability starting Intravenous Therapy (IVs).

Ability to reconstitute medications.
