Job Summary:

The Fellowship is a 12-month long position which combines mentorship, problem based learning, and supported clinical experience in an outpatient family practice setting Under the mentorship of Faculty clinicians the Fellows gradually increase the number and complexity of patients seen Specialty, Women’s Health, and Pediatric rotations add to experiential foundations in Family Practice Salary and full benefits offered

Timeline:

• 5 Positions are available each year

• Application opens November 1st

• Deadline for submitting completed application and all required criterion is March 14th

• Interviews are scheduled following the March 14th closing date

• Candidates will receive an automated e-mail after submission of application

• Applicants are notified of admission decision by early April

• Orientation starts around Labor Day


Criteria:

Applicants will be expected to submit documentation requirements for consideration into the Advanced Practice Registered Nurse (APRN) Primary Care Fellowship.

The following items are required to complete the application process for the Peak Vista Community Health Centers Advanced Practice Registered Nurse (APRN) Primary Care Fellowship Program. Any exceptions are at the discretion of the Program Director.

Completed PVCHC APRN Primary Care Fellowship Application Must be graduates of an accredited family nurse program (by orientation date -- with less than 750 hours of supervised prescriptive practice, as defined by Colorado DORA) or transitioning FNPs who demonstrate an intent to practice as a primary care providers Must have at least 2 years of RN work experience Certification in Family Nurse Practitioner Specialty  Active & Unrestricted Colorado RN & APRN license (eligible and complete by orientation date) Three (3) letters of recommendation - One (1) from a clinical preceptor, One (1) from an academic professor from a NP program, and One (1) from a professional reference. (There will not be any recommendation letters accepted that come from a peer.) Nurse practitioners transitioning from a non-family practice specialty will be considered on a case-by-case basis Ability to commit to 12-month, full-time program Complete an essay, no more than 500 words, answering the questions below and submit by email to APRNFellowship@peakvista.org: What are your professional values as a nurse practitioner? Describe what makes you an ideal candidate for the Primary Care Fellowship. Discuss your expectations and goals for the Fellowship. Tell us about your experience working with people facing access barriers and your commitment to working with the underserved population.
