Job Overview:

RWJBH is seeking a Full Time Medical House Nurse Practitioner at Clara Maass Medical Center. The nurse practitioner (NP) functions as an independent care provider in collaboration with the medical staff to provide care for surgical patients. Their advanced practice is characterized by comprehensive health assessments and management of patients with acute and/or chronic health problems. Care is provided to surgical patients in the outpatient and inpatient settings. Responsibilities include: obtain patient histories, perform physical examinations, order and interpret diagnostic tests including but not limited to radiology and lab tests, prescribe pharmacological and non-pharmacological treatments, education patients/families regarding disease process, medical intervention, and health promotion, and documentation of care provided. Additional responsibilities include but are not limited to: write admitting orders, routine orders and daily progress notes and perform clinical procedures including suture removal, administration of intrathecal chemotherapy, bone marrow biopsy/aspiration, and paracentesis. The NP communicates pertinent abnormalities to the collaborating physician. The NP serves as a source of other members of the interdisciplinary team in regards to patient and disease management.

Assisting patients with coordination of care and admission. Assisting with Discharge of patients and transition of care. Assisting with Medication Reconciliation.

Essential Functions:

Assess and manages common health problems occurring during hospitalization.Communicates with collaborating MD, in a timely manner (i.e. patient abnormalities)Coordinates discharge plan, education, and follow-up.Demonstrates ability to make effective nursing judgments including handling of crisis/emergency situations.Develops and initiates an individualized plan of care in collaboration with the members of the interdisciplinary team.Initiates appropriate referrals to disciplines relevant to the patient s care (i.e. psychosocial counseling, genetic counseling, the center for integrative medicine, nutritional counseling, hospice etc)Initiates appropriate referrals to medical consultants in collaboration with the physician.Orders and evaluates pertinent diagnostic tests appropriate to the patient s plan of care.Performs and documents comprehensive and episodic health histories and physical examinations in order to identify existing or potential health problems.Prescribes non-pharmacologic treatments (i.e. PT/OT/lymphy edema therapy etc)Prescribes, monitors, and adjusts patient medications according to medical plan of care.Provides and documents health education to patient and family regarding disease process, management of treatment side effects, medical interventions and health promotion.Provides follow-up care including routine and acute assessments and examinations. Assess progress, identify problems, and initiate preventative measures in collaboration with the physician.Serves as a resource for nursing and ancillary staff with regards to patient disease management and post operative course.Writes admitting orders, daily routine orders, and daily progress notes in collaboration with the physician.
