Overview
The Hospital at Home Advanced Practice Nurse Provider (APN) will serve in a dual capacity. The APN will take a lead role in identification and assessment of potential Hospital at Home referrals, in collaboration with the Virtual Hospitalist. The APN must be committed to providing high-quality, evidence based, patient centered care to a diverse patient population in a non-traditional setting. Additionally, the APN will conduct in-home patient visits to Hospital at Home patients.
Seeking an Advanced Practice Nurse for Hospital From Home program to work 12 hour shifts 7am to 5:30pm during the week to include weekend and holidays. Prefer: APN with 1 to 2 years of experience.
Responsibilities
1. Provides high-quality, inpatient-level acute care to diverse patient populations in their homes. 2. Performs comprehensive in-person assessments and physical exams as well as virtual assessments, and when needed, collaborates with the Virtual Hospitalist to develop and implement a plan of care; inputs orders in accordance with relevant guidelines and protocols, demonstrates understanding of evidence-based medicine; obtains additional diagnostics and specialty consultation where needed; completes clinical documentation in accordance with institutional standards. 3. Conducts review of potential patients via the electronic medical record and identifies candidates for transfer to Hospital at Home; communicates with brick-and-mortar teams; where needed, manages inbound referrals and consults to the Hospital at Home service. 4. Performs face-to-face evaluations and assessments of Hospital at Home admissions; where needed, completes admission notes and orders. 5. Describes and demystifies the Hospital at Home model for eligible patients and caregivers, including the continuum of the phases of care; responds to questions and concerns in an empathic and reassuring manner. 6. Works closely with fulfillment to understand intake capacity in order to maximize daily program patient intake; quickly addresses obstacles to patient intake; participates in creative problem-solving and flexible care plan creation. 7. Acts as an escalation point of contact for patient intake questions and troubleshooting. 8. Communicates and builds collegial relationships with clinical staff and multidisciplinary colleagues in the Command Center and brick & mortar referral sites (e.g., Emergency Department, Med-Surg floors, Urgent Care, Primary Care); provides constructive guidance to both groups. 9. Evaluates the referral and patient intake process to ensure the process is seamless, efficient, and effective for all stakeholders. 10. Collaborates seamlessly with and performs safe hand-offs to Virtual Hospitalists, Virtual RNs, and primary care physicians. 11. Demonstrates superior communication skills with patients and caregivers and practices patient-centered care, eliciting patient and family goals. 12. Promotes professional practice and a culture of safety; reports Sentinel Events and Serious Incidents; participates in quality and safety reviews, root cause analyses, and other process improvement efforts. 13. Engages in continuous process improvement and program growth efforts. 14. Ensures eligibility and assessment tools are accurate, up-to-date, and readily accessible. 15. Participates in ongoing learning and continuing education. 16. APN, uses advanced nursing knowledge to identify, prevent, and solve complex and/or recurring patient care problems; serves as a role model for professional nursing practice and maintains and practices in accordance with nursing guidelines per State and hospital regulations; performs other duties and responsibilities as assigned by the Director of Nursing. 17. Assists in development of marketing collateral and training materials for stakeholder groups including patients, caregivers, physicians, care management, and nursing. 18. Supports research efforts to advance knowledge in the field and promote evidence-based practice. 19. Collaborates with leadership to develop and implement new processes, with potential to train and onboard staff. 20. Committed to providing high quality, evidenced-based, patient-centered care to a diverse patient population in a non-traditional setting. 21. Proficient with electronic health records and demonstrated comfort with technology; comfortable navigating multiple technology platforms simultaneously. 22. Able to gather and synthesize a high volume of data and manage multiple competing priorities; excellent organizational skills with proven attention to detail. 23. Possesses excellent clinical decision-making, critical thinking, and creative problem-solving skills. 24. Adaptable, flexible, and thrives within a fast-paced, dynamic work environment. 25. Team player who also works well independently; self-starter and proactive. 26. Demonstrates superb verbal communication skills; communicates professionally with patients, caregivers, and members of an interdisciplinary team. 27. Identifies forces that impact the cost of health care and practices cost-effective care. 30. Experience with continuous quality improvement efforts and development of evidence-based protocols is desirable. 31. Other duties and/or projects as assigned. 32. Qualifications
Education, Knowledge, Skills and Abilities Required: 1. Master¿s Degree in Nursing. 2. Graduate of an NLN/AACN accredited program in nursing. 3. Minimum of five (5) years of experience with inpatient care or home-based setting. 4. Maintains clinical competencies through participating in continuing educational programs (contact hours per years), and clinical practice as mandated by Board of Nursing and national certification body. 6. Excellent written and verbal communication skills. 7. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms. Education, Knowledge, Skills and Abilities Preferred: 1. Institutional knowledge of the hospital or healthcare system is desirable 2. Certifications in specialty fields such as acute care, ambulatory care, oncology, and/or geriatrics desirable. Licenses and Certifications Required: 1. NJ State Professional Registered Nurse License. 2. Advance Practice Nurse License. 3. AHA Basic Health Care Life Support HCP Certification. Licenses and Certifications Preferred:
