DaVita is looking for a Nurse Practitioner (NP) to join our Nightingale Clinical Care Team in the Central Valley, CA market.

In this role, you will provide comprehensive healthcare across the many co-morbid chronic conditions common to chronic kidney disease (CKD) and end-stage kidney disease (ESKD) patients within a designated market or geographical region. CKD NP’s place of service is predominantly via telehealth and ESKD NP’s place of service is primarily in the dialysis clinic but may vary (e.g., clinic, telehealth, and in-person, home). The NP exercises discretion and independent judgment in the performance of professional duties and functions autonomously, yet contributes with all members of a patient’s healthcare team to provide, coordinate, and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for patients. The NP assumes a clinical leadership role and coordinates activities with the healthcare team and healthcare systems to meet the health needs of their patients. The NP will utilize an electronic health record (EHR) such as Cerner or Epic for patient documentation and scheduling, which will be reviewed by the NP Manager. You may be asked to provide services to patients outside of their assigned pod, either in-person or telephonically as needed.

Essential Duties and Responsibilities

Performs enhanced annual wellness visits or continuous health evaluations (CHEs) to identify and evaluate patients’ co-morbid conditions and identify and assist with patients’ needs including but not limited to medical, social, emotional, and financial concerns through medical record review, consultation with patient, family, and healthcare team members.

Manages the care of CKD/ESRD patients, using the enhanced annual wellness visit or CHE as a foundational history and physical, focusing on medical cost/utilization and preventing/reducing hospitalizations, and re-admissions through proactive post-discharge planning and follow-up with patients most at risk.

Leads the clinical team and ensures care management and care coordination activities complement and support medical treatment plans while integrating patients’ psychosocial and physiological needs.

Documents findings in patient medical record using an EHR system, ensuring that appropriate diagnoses are evaluated and accurately documented. Uses EHR calendar function for all work-related activities.

Communicates and collaborates with DVPs, Directors, Nephrologists, other providers, Nightingale NPs, in-center hemodialysis teams, DaVita IKC employees, and medical directors regarding the healthcare needs of the patients.

Travel: Local travel up to 100% as defined by geographical region

Nights/Weekends: Nights up to 10% of travel or as needed for completion of above tasks

Preferred Experience

0-2 years of NP Experience; experience in population health, Primary/family care, and/or nephrology a plus

Master's degree in Nursing

Demonstrated proficiency in clinical assessment, diagnosis, planning, implementing, documenting, and evaluating the needs and care of patients with chronic diseases

Ability to become credentialed with DaVita Kidney Care, Medicare, and private payers

High level of proficiency with Cerner/EPIC or similar EHR system

Proven knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes

History of dealing with confidential information and/or issues using discretion and judgment

High-level communication skills in clinical and business settings with other practitioners and upper management

Intermediate computer skills and proficiency in MS Word, Excel, and Outlook

Proven ability to lead others and drive clinical outcomes

CERTIFICATES, LICENSES, AND REGISTRATIONS REQUIRED FOR THIS POSTION

NP license; certificate of completion from an accredited NP Program

CPR certification

DEA license or eligibility to obtain within 90 days of hire

Copy of MSN diploma

Valid driver’s license and insurance within state of residence
