The NP will work closely with our physicians and multidisciplinary staff to deliver exceptional patient care. The ideal candidate will have expertise in chronic disease management, preventive care, and cardiology, as well as excellent interpersonal and communication skills. Responsibilities: Provide patient-centered care, including conducting physical exams, obtaining medical histories, and diagnosing and managing acute and chronic conditions. Monitor and manage chronic illnesses such as hypertension, diabetes, and cardiovascular diseases. Assist in performing and interpreting diagnostic tests such as EKGs and stress tests in collaboration with cardiologists. Prescribe medications, therapies, and lifestyle modifications tailored to each patient’s needs. Conduct routine follow-ups to assess patient progress and adjust care plans as needed. Educate patients and families on disease management, medications, and lifestyle changes to improve health outcomes. Collaborate with the physicians, nurses, and administrative staff to ensure seamless coordination of care. Maintain accurate and timely documentation in the EMR system. Participate in quality improvement initiatives and stay updated on advancements in internal medicine and cardiology. Qualifications: Master’s degree in Nursing from an accredited Nurse Practitioner program. Active and unrestricted Nurse Practitioner license in New York State. Board certification as an Adult, Family, or Acute Care Nurse Practitioner. Current BLS and ACLS certifications (required). Minimum of 2 years of experience in internal medicine or cardiology (preferred). Proficiency with EMR systems; experience with [specific EMR] is a plus. Strong diagnostic, clinical, and communication skills. Ability to work independently and collaboratively in a fast-paced environment.
