Position Summary: Manages and coordinates all aspects of patient care in accordance with State and Federal rules and regulations and the nursing standards based on the scope of a licensed independent practitioner.
Job Responsibilities:
Manages a clients overall care; diagnoses health problems; prescribing medications and treatments within the scope and formulary set forth by collaborating physician; treat chronic and acute conditions including injuries, infections, diabetes and high blood pressure; detect changes in patient condition and recommend further treatment options or future appointments if needed; provide after-care / ongoing care information and resources to patientsPerform initial patient screenings, patient history and physical examinations; facilitate new patient consultation; perform invasive procedures; ensures patient records are accurate at each patient appointment; administer medications; operate medical equipmentOrder, conduct, analyze and interpret diagnostic test such as x-rays, EKGs and lab workDeliver quality, physician-level care while complying with agency policies, safety regulations, and productivity metricsEnsure patient records are in compliance and up to date with accurate and current test results, immunizations, condition changesCollaborate and consult with other healthcare professionals and multidisciplinary team members as needed; provide referrals to other practitioners when necessaryServe as a consultant and resource for patients need for ongoing care information; provide counseling and guidance for any patients with critical care conditions; facilities staff, patient and family decision making by providing educational tools; acts as patients advocate for families, individuals, groups and communities; educate patients about preventive care and prescribed treatments Facilitates the patients transition within and between health care settings (admitting, transferring and discharging patients)Provide oversight, delegation and direction to RN, LPN and MA; ensure use of established procedures, policies, guidelines and standards under the disciplines scope of practiceOther duties as assignedRequired Knowledge, Skills and Abilities:

Masters Degree in Nursing Prescriptive authorization Minimum 5 years nursing experienceActive Nurse Practitioner license in Ohio Valid drivers license and current auto insuranceEssential Functions/Physical Demands:
