Nurse Practitioner or Physician Assistant - Breast Oncology
Overview:

Community Health Partners and Community Breast Specialists is seeking an Advance Practice Provider (APP) to work in an established Breast Oncology practice located in Clovis.
The APP provides routine care and management of the breast oncology patients in outpatient and inpatient setting, in collaboration with the supervising physician, and through implementation of standardized procedures.


Join our mission-driven team as an Advanced Practice Provider (APP) and play a pivotal role in transforming the care experience for breast oncology patients. In this position, you will be at the forefront of providing comprehensive routine care and management for patients in an inpatient and outpatient setting, working closely with a supervising physician to ensure the highest standards of care through the implementation of standardized procedures.
As a key member of our developing breast program, you will collaborate with a multidisciplinary team, including breast surgeons, radiation oncologists, and the Community Cancer Institute, to deliver integrated, patient-centered care. In addition, your role will also involve exciting initiatives such as clinic trials and survivorship program, supporting patients beyond treatment into long-term health. This role is not just about clinical expertise—it’s about being part of a visionary team dedicated to advancing breast health, optimizing patient outcomes, and supporting individuals at every stage of their journey while redefining the future of breast cancer care.

Responsibilities:

In this role, you will:

Perform complete/comprehensive history and physical examination, ordering and interpretation of diagnostic studies, and prescribing appropriate treatments, symptom management, adjustment of chemotherapy orders in collaboration with the attending physician.Document and maintain precise and complete records of patients, ensure patient safety, and contribute to quality and regulatory activities. Documentation of patient care/procedures per policy, and ensuring longitudinal communications with primary oncologist and referring providers regarding patient status.Other responsibilities include participating call rotation, vacation coverage, and breast program development related duties e.g. clinical trials, survivorship. Experience and Education Minimum Requirements:

Completion of a Nurse Practitioner or Physician Assistant program requiredCredentialed by the CMC Medical Staff Office as an Allied Health Professional and privileged to perform Nurse Practitioner or Physician Assistant Standardized Procedures required2 years of clinical experience preferredOncology experience preferredIf qualifying as a Nurse Practitioner, the following is requiredNP - Current State of California Nurse Practitioner License requiredNPF - Nurse Practitioner Furnishing Number required.If qualifying as a Physician Assistant, the following is requiredPA - State of California Physician Assistant License requiredDEA - DEA Controlled Substance Number requiredBLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) requiredAbility to participate in a federally funded program (Medicare and Medicaid) requiredNPC - National Certified Nurse Practitioner by ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners) preferred
