Helping Hands is expanding and looking for a Nurse Practitioner - PRN to join our Medical Network division. Overview: Under the direction and supervision of a Physician, the CRNP will provide primary medical care in an outpatient setting. Must be knowledgeable regarding the nursing process and its application, including the planning, implementation, and evaluation of nursing care. Qualifications: * Graduate of an accredited School for Nurse Practitioners * Certified as a Registered Nurse Practitioner * Current licensure by Commonwealth of Pennsylvania Essential Duties and Responsibilities: * Evaluate, examine, diagnose, and treat assigned patients. * Perform routine physical examinations as required by Helping Hands Medical Network and any facilities in which patient resides. * Assess and treat acute illnesses e.g. URIs, UTIs, rashes, etc. * Assist the physician in managing chronic diseases e.g. HTN, DM, COPD, CHF, etc. * Assess preventive immunization status and ensure that all preventive immunizations are current including influenza, COVID, etc. * Promote preventative medicine with proper patient education and counseling. * Assesses patient functional status and works to maximize quality of life through additional therapy services as indicated. * Assesses Advance Directive status and, when needed, educates patient and family on Living Wills, Powers of Attorney, DNR status, POLST and other mechanisms to ensure compliance with patient wishes. * Order, review, and interpret any lab or radiologic studies obtained on the patient. * Assess any emergency situation that may arise and direct the problem to the proper facility and/or physician. * Have a physician available for consultation at all times whether by telephone or in person. * Maintain comprehensive, legible, and timely documentation of all care provided whether on paper or via Electronic Health Record. * Participate in quality assurance, infection control, and performance improvement activities as initiated by Helping Hands Medical Network or facility in which patient resides. * Participate in 24/7 on call responsibilities along with other members of the medical staff. Other Jobs Functions: * Works with diverse social and economic situations without imposing their own values. * Maintains professional boundaries. * Will be responsible for holistic care of patients. This encompasses the psychosocial, developmental, cultural and spiritual needs of the individual. * Demonstrates understanding of home health and palliative care philosophy. * Will represent the patient’s best interests at all times while maintaining their dignity throughout treatment and care. * Assists with orientation of new staff as assigned. * Familiar with adult protective service reports. * Maintains up to date knowledge in infection control precautions and isolation procedures. Required Skills, Abilities and Knowledge: * Excellent communication, time management and organizational skills. * Must be able to function independently with critical thinking skills while collaborating with other team members. * Must be self-directed and demonstrate leadership skills, along with excellent written and verbal skills. * Knowledge and understanding of agency policies, HIPPA standards, OSHA policies, applicable Conditions of Participation requirements, and applicable accreditation standards. * Has the ability to adapt to rapidly changing priorities and requirements.
