Summary
Summary: Performs comprehensive medical and nursing assessments to be used in diagnosis and treatment of patients. Serves as an advisor, resource, preceptor and leader for other members of the patient care team. Responsiblities Provides direct and indirect patient care optimizing patient safety. Works proactively and in collaboration with other team members. Evaluates patients, obtains complete health history from patient or family. Performs comprehensive appraisal including physical examinations and psychological and developmental/situational evaluation. Provides medical advice and assistance and crisis intervention by telephone when required. Establishes medical diagnosis for common short term or chronic stable health problems. Prescribes drugs in collaboration with a licensed physician. Performs various diagnostic procedures such as bacteriological stains, smears, cultures, drawing blood, and urinalysis. Orders laboratory tests, x-ray studies or other diagnostic tests as appropriate. Establishes therapeutic or corrective measures as related to specific illnesses or anticipatory guidance. Refers patient to appropriate licensed physicians or other health care providers. Coordinates care with specialty clinics, physicians, social agencies, schools, governmental agencies, etc. on behalf of the patient. Maintains and reviews patient records. May track and report patient data to monitor clinical outcomes Works in a professional and collaborative manner with all staff and physicians in the delivery of patient care. Displays a commitment to the value of teamwork by building strong relationships with patients, families, coworkers and the surrounding community. Cooperates and collaborates to perform duties promoting a unified and spirited workplace. Displays an understanding and awareness of, and performs duties in accordance with, the Hospital's Mission and Vision statements. Performs related duties as required or assigned. Participates in the staff and patient education process. Develops patient education material promoting current clinical practice guidelines. Develops educational programs for house staff, nursing and medical students, community agencies, in-service instruction, etc. Attends meetings such as patient care conferences, team meetings, staff or departmental meetings, grand rounds, training programs, seminars, etc. Incorporates appropriate material into clinical practice operations. Instructs and counsels patient and/or family concerning health care matters such as use of medications, diet instruction, disease prevention, hygiene, etc. Provides emotional support for patient or family members stressed by illness. Maintains certifications as a Nurse Practitioner through attendance at accredited continuing education conferences. Supports and demonstrates regulatory compliance related to quality standards and expectations, policies and procedures. Adheres to all Hospital infection control and safety policies and procedures. Identifies opportunities and makes recommendations for process improvements resulting in optimized patient care Experience: BLS Nurse Practitioner License#APP
