Description:In collaboration with the attending physician, prepares and records patient history, develops and implements a plan of care, participates in daily rounds on assigned patients and documents the pertinent symptoms, findings, diagnoses and care plans. Utilizes Powerchart and Powernotes for order entry and documentation. Participates in multidisciplinary rounding, responds to coding and documentation queries and participates as requested in quality and safety initiatives. Functions overall in accordance with the House and Service Coverage Plan. Accountable to the Clinical Department of Chief and Director of Outcomes Management.Job Description

The Advanced Practice Practitioner for Primary Care collaborates with the physician and other health care professionals to provide medical services including direct patient care, medical record documentation and follow-up care to patients. Additionally, the NP/Care Manager functions as a facilitator of interdisciplinary collaboration and care continuity across care settings within the ambulatory environment. Interaction with the patient and St. Mary Mercy Medical Group(SMMMG) physician may occur at various touch points across the continuum, based on assignments, beginning while the patient is in the hospital and extending to include follow up phone calls and patient visits within the various medical practices within the SMMMG. This role is focused on provision of information and support for the patient and SMMMG physicians or affiliated specialists in order to ensure effective care transitions, improved self-management skills and enhanced patient-practitioner communication. Specific performance duties will be based on the population assigned to SMMMG with expectations of being able to provide cross-coverage for SMMMG primary care physicians.  The Physician Assistant or Nurse Practitioner operates within the standards of care and scope of practice as defined by the State of Michigan.

Masters degree in Nursing required. State of Michigan Nurse Practitioner license required. Certified through the National board depending on specialty.  2-5 years if RN experience required; previous Nurse Practitioner experience preferred. Knowledge of patient care procedures and organizational policies related to position responsibilities. Must have a high level of proficiency using a computer system, with the ability to efficiently utilize EMR templates, NextGen and Microsoft Office (Outlook, Word).
