**Community Information** **Fairview** is seeking a **Nurse Practitioner** to join our **Wound Care** team at both M Health Fairview - Wound Healing Institute - Edina **,** as well as at our East Vascular Center, located in the M Health Fairview Clinics & Specialty Center in Maplewood. Work in the capacity of an attending provider in an outpatient specialty services setting including patient care management, wound and vascular care, ordering and interpreting diagnostic tests, performing diagnostic and therapeutic procedures, prescribing medications and discharge planning. **Practice Details:** + **Collaborate, consult and communicate** with a multidisciplinary team of healthcare providers and staff throughout the continuum of care. + 3 days/week in **Edina** , 2 days/week in **Maplewood** . + Average of **12-18 patient encounters per day.** + **Openings** : 1.0 FTE (Full Time) + **Fully integrated** Electronic Medical Record (EPIC).
