The Nurse Support Coordinator supports the location’s Director, Clinical Services and team members in the completion of daily activities to optimize the clinical staff’s time in caring for patients.
Responsible for obtaining authorizations, benefits coverage and requested updates for continuation of services. Notifies Director, Clinical Services (DCS) as needed to ensure necessary information and documentation is obtained.Completes the intake referrals according to established processes ensuring complete, accurate timely information.Acts as a clinical intermediary for the field staff including but not limited to: 6. Updates or changes in physician ordersEnsures the processes for physician orders (485’s), recerts, OASIS assessment data entry, updates and changes are completed accurately and in a timely manner. Acts as a liaison with corporate Revenue Cycle Department to resolve questions and issues provide necessary documentation,etc.Takes referrals and completes the intake according to established processes ensuring complete and accurate information.Responsible for obtaining authorizations, benefits coverage and requested updates for continuation of services. NotifiesAssists in training new personnel as requested by management. Assists in identifying clinical training needs.Acts as a resource for external customers, including but not limited to, physicians, referral sources and discharge planners/social workers.Performs client visits as needed and directed by managementSupports coordination of care through case conferences and other activities for interdisciplinary care.All other duties as assigned.Qualifications
