Cassia exciting job opportunities to work as a Nurse Transitions Liaison! In this role, you will build relationships between the patient/family and the staff of both the discharging care location and the Cassia community or service system on behalf of an individual who is transitioning from one care system to another. As an advocate for patients/families during the Cassia pre-admission process, you will see the process through until the patient is discharged from an inpatient hospital, emergency department, clinic, or other medical facility and has safely entered a Cassia Care Community or service system.
The Nurse Transitions Liaison primarily supports Cassia’s skilled health care centers and will also represent the Cassia continuum of care at large, particularly within the hospital system and our established referral network. This extended reach would include but is not limited to assisted living, open circle adult day, Home Care, Hospice, pharmacy services and more.
This is a Full-Time position with a generous benefits package. You will receive support from our experienced Sales and Marketing leadership team to implement creative strategies as you forge relationships. In this role, you will work at Cassia sites, hospitals and remotely.
Responsibilities:
The Nurse Transitions Liaison would ideally possess a current nursing licensed (LPN or RN) with skilled care experience within senior care, or, an individual with 5+ years of Case Management, SW, or Skilled Care Admissions experience. Other requirements include:Ability to travel and work in a variety of healthcare settings such as hospital, clinics, skilled nursing facilities, rehabilitation centers and prospect homes.Must be self-directed and self-motivated. Possess comprehensive clinical understanding of skilled nursing, assisted living, home health, hospice and adult day.Must demonstrate exceptional verbal, written, active listener and interpersonal skills.Have knowledge of electronic health records. Must have excellent organizational skills and be an effective time manager and multi-tasker.Practical experience in a skilled nursing, clinical, or acute care setting preferred.
