Nursing Assistant II (CNA)- Imaging Unit .5FTE, 7a-7p
Major Responsibilities: Provides general patient care duties that are specific to the patient population served as directed by the registered nurse.     9) Removes foley catheter     1)Assists with personal hygiene care such as making occupied beds, complete or partial bed bath, oral and denture care, shave, back rub, and skin care.     2)Assists patient with activity orders which may include the use of therapeutic assistive devices, proper positioning, range of motion, and special transferring as indicated.     3)Assists with toileting needs of the patient including care of the incontinent patient.     4)Responds to patient and family requests and escalates issues to the registered nurse.     5) Performs purposeful hourly rounding as evidenced by documented rounding logs.     6)Removes IVs and saline locks as directed, provides site care and proper disposal of IV and tubing.     7)Performs phlebotomy and collects and prepares specimens, including stool, urine using say it out loud process for proper identification and labeling. Performs point of care testing.     8)Ensures special patient supplies an appliances are provided and maintained such as TED hose, sequential compression devices, and K-pads. Ensures accurate and effective management of the patient's medical records to promote coordination of patient care and optimal coordination of the plan of care     3)Monitors patient medical record for new and stat orders and alerts the nursing staff     4)Verifies that medical records contain complete and accurate patient demographic information. Determines completeness of the medical record and prepares for final assembly prior to patient discharge and or coordinates printing and copying of the record prior to transfer to other facilities     5)Verifies appropriate patient identification process and registration in the computer system. Identifies any discrepancies in assignment of level of care and takes appropriate steps to reconcile.     6)Assemble all non electronic portions of the MR in the proper sequence, ensuring that labels are correct     7)Processes patient charges for multidisciplinary services as appropriate and ensures patient procedures and tests are scheduled in a coordinated sequence.     8)Notifies appropriate team members regarding outstanding orders and communicates pertinent and timely information to other departments and staff.     1)Serves as the unit expert on all aspects of maintenance and utilization of the EMR     2)Supports the education needs of clinical users regarding the use and application of the EMR and other systems. Establishes and maintains positive customer relations and contributes to a harmonious work environment     1)Promptly and courteously acknowledges and responds to internal and external customers who approach the nurses station. Facilitates the customers' request and demonstrates positive customer service behaviors.     2)Promptly acknowledges and/or communicates to staff, call lights which have been activated.     3)Demonstrates proper phone etiquette and function     4)Coordinates incoming and outgoing calls, including recording and reporting of pages and messages.     5)Fosters positive communication between all members of the health care team.     6)Maintains cleanliness and organization of immediate work area.     7)Promotes hospital recycling efforts by proper disposal of printer forms/results. Shreds patient health information and recycles. Participates in the admission, transfer, and discharge process of all patients under the supervision of the registered nurse or designee.     1)Prepares patient room for admission per unit standard.     2)Introduces him/herself to the patient and family; assists patient with changing into a patient gown.     4)Strips linen from bed and removes patient equipment and linen at discharge.     5)Ensures patient identification band placement, takes vital signs, height, and weight, and other unit based tasks such as belongings check.     6)Assists with patient transfer or discharge as assigned, ensuring that the patient belongings are accounted for and transports patient to discharge door as needed.     7)Assists in providing post-mortum care and transport to the morgue as needed.     3)Orients patient/family to room, including physical layout of room, call light, bed functions, TV, telephone and process for ordering food and other functions specific to the unit. Performs tasks related to meeting the nutritional needs of the patient.     1)Reviews dietary order to ensure patient is receiving proper diet, initiates reordering, as needed, under the direction of the registered nurse.     2)Assists patient with meals, including preparing and feeding as needed such as patients on modified diets as well as dysphagia precautions. Passes water and nourishments. .     3)Provides proper tray set-up and patient positioning with consideration to special needs     4)Monitors and documents intake/output and provides accurate information for calorie count at the point of care. Documents in the electronic medical record all care provided, in real time and at the point of care.     1) Performs tasks related to maintaining the integrity of patient care standards and efficient unit operations.     1)Demonstrates ability to respond to emergency situations.     2)Maintains a safe patient environment based on age, deficits and developmental needs; practices appropriate use of restraint policy, when necessary, under the supervision of the registered nurse.     3)Maintains and develops technical skills by attending mandatory education, assists in Continuous Quality Improvement process through monitoring standards; assists in the evaluation of new products and participates in unit meetings.     4)Practices within the policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with the patients in isolation. Responsible for performance development and growth of job skills within the changing health care environment.     1)Serves as a coach and mentor to members of the healthcare team.     2)Demonstrates the aptitude to evolve and incorporate new technologies and/or processes into the work environment.     3)Continually improves personal, technical, and professional skills     4)Identifies possible impact and /or barriers with regard to implementation of upgrades or new systems     5)Attends training sessions and meetings as requested

Education/Experience Required:Certified Nurse Assistant OR Nursing Student with first clinical completed OR Paramedic OR LPN with current IL license. Plus one year work experience preferred in computers, variety of programs, and data bases.

Knowledge, Skills & Abilities Required:Good communication skills. Courteous and professional interpersonal skills. Basic math calculation. Proficient computer skills. Basic Life Support Certification within 90 days of hire.

Physicial Requirements and Working Conditions: Ability to demonstrate a commitment to patient-focused care. Ability to work in a fast paced environment with established time constraints and in difficult situations. May be exposed to hazardous materials and life-threatening diseases. Ability to perform lifting/transfer activities related to patients (up to 50 lbs. may be required). Ability to work designated shifts including weekends and holidays. Ability to transport objects from one location to another. This position needs to recognize needs and behaviors of specific age groups of patients treated. Ability to successfully complete hospital based training for EMR.If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
