Position Summary

Accurate Home Care has an opening for an Oasis Educator who will ensures patient satisfaction through thorough oversight of training program, education on OASIS, QA and providing quality patient care.to lead our home health program. At Accurate Home Care, our Home Health team aids in empowering our patients to stay at home and recover. This strong team of home health professionals manages the care of our patients while building relationships and ensuring a positive patient experience.

What will I be doing?

Completes patient visits as needed as per direction of the Director of Home Health. Plans, coordinates, delivers, and oversees the delivery of outcomes of required healthcare related services/training programs on a routine basis Participates in the implementation and oversight of nursing an d home health aide services. Review data entry in medical record system to ensure accuracy for billing and compliance. Conduct patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans, and discharge. Timely documentation of visits and supervisory notes in accordance with governing body. Participates with Director of Home Health in training both new hire and existing qualified personnel. Oversight of visit staff to ensure adequate field orientation, mentoring, in-services and evaluations. Understand and comply with policy and procedures set forth by AHC. Maintain compliance with and stay abreast of changing county, state, and federal regulatory initiatives affecting home health care, including Medicare Conditions of Participation.
