**Overview**
Dignity Health’s Pacific Central Coast Health Centers (PHC) is a non-profit Community clinic organization comprised of more than 40 health centers located primarily from Ventura to Templeton spanning the coast. Our experienced physicians and advanced practice providers offer a wide range of primary and specialty care services. PHC is a part of Dignity Health Central Coast’s award-winning network of outstanding hospitals imaging centers laboratories and post-acute services.
**Responsibilities**
In keeping with the Pacific Central Coast Health Centers philosophy the CPSP (Comprehensive Perinatal Services Program) Educator manages health education assessment and reassessments interventions referrals and follow-up care for clinic patients with CPSP Case Managers and Providers.
**Qualifications**
**Minimum Requirements:**
+ Minimum two (2) years perinatal case experience management; or equivalent combination of education and experience.
+ Knowledge of perinatal services and Comprehensive Perinatal Services Program (CPSP) guidelines
+ Good verbal and written communication skills.
+ Good interpersonal and organizational skills.
+ Works effectively with a variety of diverse populations.
+ Can work with a team to plan develop and implement programs and services to promote community health education and health promotion thus improving the health of the individual patient and the community.
+ Flexible; handles several projects concurrently.
+ Strong attention to detail.
+ Tactful sensitive and possesses knowledge of cultural differences when working with clients.
+ General office experience and basic computer skills.
**Preferred Qualifications:**
+ Bachelors degree in Social Services nursing or other related health field
+ Bilingual English/Spanish preferred.
**Pay Range**
$25.25 - $36.61 /hour
