This is currently a part time position with a possibility of being full time.
This position is responsible for OT evaluations, and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The OT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The OT assumes responsibility for participants treated by the OTA and OT Aide.

Required Qualifications:
Bachelor's Degree - Occupational Therapy. Oregon Occupational Therapist License upon hire. National Provider BLS - American Heart Association within 30 days of hire.Preferred Qualifications:
Master's Degree - Occupational Therapy or, OTD (Occupational Therapy Doctorate). 1 year - Experience working as an occupational therapist in a geriatric setting.Why Join Providence?
