Occupational Therapist
Job Summary: Occupational Therapist (OT) to manage and implement a Home Modifications Grant Program for older adults. The OT will work closely with clients, caregivers, contractors, volunteers, and healthcare professionals to ensure that the home modification needs of older adults are met in a safe, effective, and efficient manner. This role will involve assessing client needs, coordinating home assessments, and ensuring that modifications are completed in accordance with healthcare standards. Work Environment: + The role requires regular visits to client homes, collaboration with contractors, and occasional office work for grant management and documentation. + Flexible work hours may be offered depending on client needs and project deadlines. Job Responsibilities: + Client Assessment & Care Planning: + Conduct comprehensive assessments of older adults’ functional abilities and home environments to identify the need for modifications. + Recommend appropriate home modifications (e.g., ramps, grab bars, wider doorways) to enhance independence and safety. + Collaborate with other key staff to determine how to achieve the modifications needed. + Grant Management & Documentation: + Work with the grants team on the administration of the Home Modifications Grant, ensuring compliance with eligibility criteria and program guidelines. + Track grant expenditures and prepare reports for stakeholders, including funding agencies, and ensure that funds are used appropriately. + Coordination with Contractors & Service Providers: + Collaborate with contractors and vendors to obtain quotes, select service providers, and ensure modifications are completed according to specifications. + Coordinate timelines, ensure quality assurance, and address any issues or delays that may arise during the modification process. + Serve as the point of contact for clients during the modification process, providing updates and support as needed. + Education & Support: + Provide training to clients and their families on how to use newly modified spaces safely and effectively. + Educate clients about the benefits and importance of home modifications in promoting health and preventing accidents. + Offer ongoing support to clients via other programs and services offered by The Jewish Federation and our family of agencies, after modifications are completed, including follow-up assessments and adjustments if needed. + Collaboration & Reporting: + Work closely with multidisciplinary teams, including healthcare providers, social workers, and other community organizations, to ensure holistic care and service delivery. + Maintain accurate records of all assessments, interventions, and modifications in compliance with regulatory standards. + Prepare and submit progress reports, data analysis, and other required documentation for grant funders and internal stakeholders. Required License: Current state licensure as an Occupational Therapist. Required Education & Experience: + Bachelor's or Master's degree in Occupational Therapy from an accredited institution. + At least 3 years of experience as an Occupational Therapist, with a strong background in home assessments, aging in place, and working with older adults is preferred. Strong understanding of home modification strategies, assistive technologies, and environmental adaptations. + Excellent communication and interpersonal skills to work effectively with clients, families, contractors, and healthcare teams. + Ability to work independently, prioritize tasks, and meet deadlines. + Proficiency with Microsoft Office Suite and other relevant software.
