Occupational Therapist - HOME HEALTH

As a key member of our Home Health Rehab team you will further our mission by:

Serving as a liaison between Community Medical Centers and the community by initiating, intervening, and evaluating nursing services to patients in their home.

Implement the care plan, including patient and family education, in collaboration with other team members.

In this role some of your key responsibilities would include but are not limited to:

Provides occupational therapy services to patients including assessment, treatment, program planning and implementation; along with related documentation and communication, with objective of enabling patient to achieve optimal functional independence.Qualifications:Education• Bachelor's Degree required• Completion of internship program in Occupational Therapy required Experience• Working knowledge of standard treatment and evaluation tools required• 2 years of therapy experience preferred Licenses and Certifications• OT - State of California Occupational Therapist License required• DL - Valid California Driver's License and driving record acceptable to Community Medical Centers required• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
