implements individualized treatment goals and plans in alignment with the patientcaregiver goals Determines treatment strategies and delivers Occupational Therapy services that promote health and wellness prevent injury or disability improve and develop self dependency and maintenance The Occupational Therapist works in collaboration with an interdisciplinary behavioral health team to reduce symptoms of mental illness through engagement in healthy roles and routines in order to improve productivity cognition physical sensory perceptual psychosocial and emotional wellbeing in a variety of contexts and environments in order to maximize function in the areas of daily living This role encompasses evaluation skills and techniques analysis of patients and application Occupational Therapy to maximize the patients functional independence May work in a Behavioral Health capacity in an Inpatient or Outpatient environmentKey responsibilitiesUpon Physician referral performs thorough evaluation of patientrsquos level of function Performs evaluations regarding sensory modulation sensory based coping skills perceptual cognitive motor function and functional abilities of daily living Recognizes physiological psychological and age related changes in the patient Consults with other Health Care Team members as necessaryDevelop an individualized patient centered treatment plan of care Evaluate care plan on an on going basis revising as necessary in accordance with patientrsquos care needs Establish measurable goals with the patientfamily based on appropriate patient care protocols and
