Provides medically prescribed treatment concerned with improving or restoring ADL functions that have been impaired, reduced, or permanently lost by illness or injury. Also provides necessary consultation and teaching to participants/family/caregivers and the interdisciplinary team.
ResponsibilitiesJob duties specific to this position:
Provides instruction and consultation to participants, families and other members of the interdisciplinary team.Supervises paraprofessional staff in the performance of their duties as they relate to Occupational Therapy and documents accordingly.Develops and implements a rehabilitation plan of care to promote, restore or maintain participant’s optimum level of function in conformance with provider’s orders and care plan goals.Maintains necessary record keeping functions pertaining to participant care.Evaluates participant’s level of function through an occupational therapy evaluation and develops a restorative or maintenance plan.As member of interdisciplinary team performs pre enrollment assessments of potential enrollees as needed; performs initial evaluation on new participants; develops a treatment plan as appropriate in cooperation with participant, family, and team members; participates in periodic reassessments when participant is receiving or in need of occupational therapy services; additionally completes annual assessments; attends morning meetings and Intake & Assessment meetings; participates in family meetings, nursing home care planning meetings, and clinical case conferences as appropriate.Conducts an ongoing evaluation of the participant’s rehabilitation progress to assess the effectiveness of treatment.Monitors participant response to OT interventions and modifies treatment, as indicated.Participates in the development and revision of the participant’s Plan of Care as necessary as a member of the Interdisciplinary Team.Provides functional enhancement through selection of task oriented therapeutic activities, training in activities of daily living, instrumental activities of daily living, functional mobility, teaching of compensatory techniques, and home assessment/modification.Participates in QAPI activities; collects data and participates on task forces as requested; member of Falls committee.Exercises judgment based on acquired knowledge of program policy and procedures when performing Occupational Therapist functions.Provides guidance in the selection and used of adaptive devices, DME equipment, and supplies; may design, fabricate and fit adaptive equipment as needed.Promotes professional growth, the profession of occupational therapy and the PACE program, when possible, by participation in professional associations, educational conferences, volunteer activities, etc.Demonstrates adherence to all compliance policies and procedures. Is responsible for promoting and fostering compliance in the workplace.Ensures the highest quality of care by performing responsibilities according to the highest professional standards.Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and can verbalize their role during an event.

Qualifications Bachelor’s Degree or Master’s Degree from an accredited program in Occupational Therapy.NYS license to practice as an Occupational Therapist. Registered with the American Occupational Therapy Certification Board.A minimum of one year experience working with a frail or elderly population.Strong interpersonal and communication skills.Demonstrated clinical competency.Valid driver’s license and a reliable vehicle.Proficient in MS Office and able to learn new software rapidly.An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
