The Home Health Field, Occupational Therapist provides **hands-on care** in the home for a group of patients consistently using the nursing process and clinical standards and protocols. **Responsibilities include** : + Responsible for the assessment, planning, implementation, and evaluation of all occupational modalities to patients. All duties and standards within this document will be performed according to established policies, procedures and guidelines within the department and the organization.
