The occupational therapist is responsible for assessing, planning, implementing, and evaluating occupational therapy interventions for residents within the skilled nursing facility. They collaborate with interdisciplinary teams to promote functional independence, improve mobility, enhance cognitive abilities, and facilitate participation in daily activities.

Responsibilities - Occupational Therapist Pool:
Conduct comprehensive occupational therapy evaluations to assess residents' physical, cognitive, and psychosocial functioning.Develop individualized treatment plans based on assessment findings, resident goals, and evidence-based practice.Provide skilled occupational therapy services to residents, including therapeutic exercises, functional activities, adaptive equipment training, and environmental modifications.Monitor residents' progress and modify treatment plans as necessary to achieve optimal outcomes.Educate residents, families, and caregivers on therapeutic techniques, safety strategies, and home exercise programs to support continuity of care.Document occupational therapy evaluations, progress notes, and discharge summaries in accordance with regulatory standards and facility policies.Collaborate with interdisciplinary team members, including physical therapists, speech-language pathologists, nurses, social workers, and physicians, to ensure comprehensive care coordination.Participate in interdisciplinary team meetings, care conferences, and discharge planning to facilitate seamless transitions of care.Adhere to ethical and professional standards of practice, including maintaining confidentiality and respecting residents' autonomy and dignity.Requirements:
