Onboarding Specialty Infusion

Job Details:

The Onboarding, specialty infusion, team members are responsible for onboarding referrals, coordinating documentation, communicating with the referral source, managing incoming faxes and assist with coordinating home health for our specialty infusions.  This role also acts as a single point of contact for designated tasks, teams and referral sources.

Responsibilities

Obtains the patient referral via phone, fax, email and/or EMR.

Documents the referral details by creating the patient information sheet.

Identifies and documents the patient’s team assignment.

Creates patient entry for delivery management.

Meets turnaround expectations for specialty infusions as directed by the business.

Enters/updates the patient profile, progress notes and home health information.

Acts as liaison between internal teams and the referral source.

Receives incoming calls, taking patient markers for new referrals.

Uploads documents to the patient’s medical record.

Manages electronic fax folder, determining the appropriate destination for the file.

Coordinates home health needs for the patient.

Acts as the team point of contact for assigned referral sources and/or therapies.

Assists with complex referrals/home health cases.

Trains/mentors new team members.

Assists Intake nurses with faxing and copying and distribution as needed.

Performs in accordance with system-wide competencies/behaviors.

Performs other duties as assigned.

Good verbal and written communication skills.

Computer literate, with the ability to utilize standard software applications including Windows and MS Office Suite (Outlook, Word, and Excel).

Good organizational skills with attention to detail and ability to multitask.

Requirements

HS diploma or GED required.

2+ years of experience in a physician practice, ancillary provider, or other relevant healthcare setting required.

Excellent communication, interpersonal and organizational skills required.

Ability to prioritize and handle multiple tasks.

Knowledge of medical terminology required.

Proficient computer skills and Microsoft Office Products required.
