Oncology Care Coordinator

Under the guidance of the Director, Cancer Center, you will be responsible for coordination of multidisciplinary clinic programs and care continuum for patients diagnosed and/or to receive therapy at the Medical Center. **A caregiver in this position works from 8:00AM - 5:00PM.** A caregiver who excels in this role will: + Coordinate multidisciplinary clinic programs, program development and services. + Navigate the care continuum for patients by collaborating with the Medical Director and act as a "compass" for patients diagnosed and/or receiving therapy. + Ensure timely scheduling of diagnostic work-up, results reporting and communications between patients and the multidisciplinary team. + Promote the multidisciplinary team process and ensure patient receives necessary referrals and follow-up. + Provide efficient and effective care for patients and families and keep current with emerging technologies and treatment trends. + Coordinate cancer related patient/staff education programs, projects, services and groups. + Assist with community events, evaluate the results and continue outcomes research. + Compile and analyze Quality Improvement data and benchmarking. + Complete mandatory education and training to maintain organization and department specific competencies and requirements and certification/licensure. Minimum qualifications for the ideal future caregiver include: + Current licensure as a Registered Nurse (RN) in state of employment + Basic Life Support (BLS) Certification + Any registered nurse or advanced practice nurse must obtain a cancer-specific certification or demonstrate ongoing qualifying education within the timeframe of the facilities accreditation cycle + Knowledge of oncology prevention diagnosis disease process and symptom management + Program completion in Professional Nursing + Five years of nursing experience including two years in oncology Preferred qualifications for the ideal future caregiver include: + Bachelor's of Science in Nursing (BSN) + Experience in case management mental health program development and community health/home care + Oncology Certified Nurse (OCN) Our caregivers continue to create the best outcomes for our patients across each of our facilities.
