Oncology Social Worker - Enfield/Springfield
**Employment Type:** Full time **Shift:** Day Shift **Description:** **Trinity Health Of New England** is looking for a **Licensed Certified Social Worker (LCSW** ) to join our Ambulatory Oncology Team. The Social Worker (LCSW) will support our two locations at **Mercy Medical Center** and **Radiation** **Oncology in Enfield, CT.** The LCSW will assesses and assists in addressing and meeting the psycho-social needs of cancer patients and families, enabling patients to utilize medical care and other supportive services to achieve their optimum level of health and function. **You Will Be Responsible For:** + Assesses and counsels’ cancer patients/families referred to the Cancer Center. + Provides consultation to physicians and staff in ambulatory discharge planning, from treatment, survivorship services, to psychosocial distress assessment. + patient centered counseling and consultative responsibilities required based on individual needs assessment. + Communicates via written documentation in the clinical record and through verbal discussions, all pertinent information on patient status to ensure that the team understands patient’s circumstances relative to achieving the goals of the treatment plan and survivorship program. **Requirements:** + Education: master’s degree Required + Valid social work license from the Commonwealth of **Massachusetts** , Licensed Certified Social Worker **(LCSW)** required. + Valid Social Worker License from the state of **CT** , Licensed Master of Social Work ( **LMSW** ) required. + Two (2) years of related professional experience required. + Ability to communicate effectively both orally and in writing.
